Liability Waiver and Media Release

I understand that there are hazards involved in dance activities, including injuries sustained from self, fellow dancers, studio floors, or equipment used (ie. ghungru). I agree to release and hold Katha Dance Theatre, their teachers, staff, board, and volunteers, and the studio owners harmless from any or all damages, losses, claims, suits, actions, expenses, or liability for any incident or injury to myself or any personal property, which may directly or indirectly arise from my participation in KDT classes, activities, shows, or programs associated with these entities. I acknowledge the dangers and risks involved, and I agree to assume all risks and dangers associated therewith. BY CHECKING THE BOX ON THE CLASS REGISTRATION PAGE, I (the student or legal guardian of student) ACCEPT THE TERMS OF THIS WAIVER.

I give permission to Katha Dance Theatre to use any photographs taken of myself participating at Katha Dance Theatre events (including classes, performances, workshops, etc.) in all forms of media, for any and all promotional purposes including advertising, publicity, display, audiovisual, exhibition, commercial or editorial use. I understand that the term “photograph” as used herein encompasses still photographs, audio recording and motion picture footage. This permission includes any photographs of myself at Katha events, regardless of whom the photographer was (i.e. official KDT-designated photographer, fellow participant, or audience member). I further consent to the reproduction and/or authorization by Katha Dance Theatre to reproduce and use such photographs for use in all markets.
